As libraries these days have library software, what is the level of automation being used? Are Institutions spending on RFID for their libraries?

Thank you Mr.Mukesh for your valuable inputs. To cover the first point I would like to reiterate that RFID is being introduced in libraries all over the world because of the huge benefits accrued viz. anti-theft feature and quick stock taking thereby reducing losses of assets; very quick retrieval of data, no line of sight required, dynamic location tagging, paper less working ,etc. If RFID is implemented in a Library by a qualified and experienced implementer there are no errors. He would have taken care of all factors to be ticked off before going live. Maybe you can share the type of errors experienced by you or the Institution you mention. As to the debate between Open Source software vs. Commercial Software, School of thought on both sides exist. There is no clearcut answer. But one prominent point which out-weighs any other consideration  is whether the Institute has a qualified and experienced Software person who can keep customizing and providing support. He should also understand RFID. So there are places where Open Source applications have succeeded while elsewhere it had to be called off.
